Use the narrowest type supported explicitly by the source:

condition: diagnosis/disease/problem mention
symptom: patient-reported symptom
sign: clinician-observed non-laboratory clinical sign
lab: laboratory measurement/result
vital: vital sign or basic physiologic measurement
medication: drug/medication mention or structured medication row
procedure: operation/procedure/intervention
imaging_finding: radiology/imaging observation or impression
pathology_finding: pathology finding/diagnosis
allergy: allergy/allergen/adverse allergy entry
device: clinical device
encounter: explicit encounter/admission/discharge event
observation: explicit clinical observation that cannot safely be typed more narrowly

Do not infer a disease from a symptom, a treatment from a diagnosis, or a causal relation in D005.
