Rows are routed using the mapped Standard Concept domain_id.

Supported domains: Condition, Drug, Procedure, Measurement, Observation, Device.

One source fact may generate multiple rows because OMOP Maps-to is one-to-many.

Unmappable but explicitly accepted facts may use standard concept_id 0 in a locally selected fallback domain. Such rows are flagged for D014.

D013 never invents missing Type Concept IDs. Missing type concepts are staged as 0 with a high-severity warning.
