v0.2.0 修复发布
本阶段诊断合并核心逻辑保持原样；本轮真实子集无诊断事实，no_patient_problems 为合理空分支，不代表完成真实诊断验证。

D009 reconciles D006 condition mentions into longitudinal canonical problem objects without terminology coding.
Design goals
- preserve every source fact and state transition;
- historical is not treated as resolved;
- suspected is not treated as confirmed;
- family history never becomes the patient's problem;
- exact/whitelisted lexical variants may auto-cluster;
- ambiguous synonyms require explicit review;
- canonical labels must come from observed member facts;
- zero network and zero third-party runtime dependency.
Run python scripts/self_test.py for deterministic regression tests.
当前版本：v0.2.1
本模块既有运行逻辑保持；本次与其他模块统一发布0.2.1，并执行原有回归。不得仅凭版本号推断新增临床能力。
