Inputs

D006 consumes only finalized D005 ClinicalFactDraft records and the de-identified D004/D003 artifacts referenced by their manifests.

Outputs

contextualized_facts.jsonl: one context-resolved record per D005 fact; fact identity and provenance are preserved.
state_transitions.jsonl: conservative exact-surface state updates with explicit from/to fact IDs.
context_resolution_manifest.json: aggregate counts only; no copied clinical prose.
context_review_queue.csv: fact IDs and reason codes only.

Invariants

D006 never changes concept.surface_text.
D006 never assigns terminology codes.
Historical mention is not equivalent to resolved disease.
Family experiencer cannot silently become patient experiencer.
Pseudo-negation such as 不能排除 cannot be interpreted as negation/refutation.
Medication action is not finalized here.
Any unresolved ambiguity blocks ready_for_domain_normalization.
