结构化状态使用精确枚举，不再用 substring：unconfirmed 不等于 confirmed，inactive 不等于 active。矛盾状态保留 unknown/review。
D-series clinical data processing Skill 006.
Purpose: turn D005 mention-level ClinicalFactDrafts into context-aware facts by resolving negation/polarity, certainty, temporality, experiencer and explicit clinical status while preserving provenance and concept identity.
Key examples:
否认胸痛-> negative / confirmed不能排除肺栓塞-> positive / possible既往心肌梗死-> historical, not automatically resolved父亲患2型糖尿病-> family experiencer- later
不支持肺栓塞-> refuted, and may form a transition from earlier suspicion if structured times are reliable
D006 is local-only, standard-library-only, does not map terminology, does not merge synonyms, and does not finalize medication actions.

当前版本：v0.2.1
读取/最终化时重查D005对应隐私收据。否定cue按mention前后位置及转折边界定作用域，排除无诱因/未缓解这类修饰对象不同的cue，并记录排除原因与offset。其父/其母等指代归family；双重否定不确定时review。confidence_kind明确为uncalibrated_rule_score。
